What a pro does

The plumber gets under the house before anything else, because in the mountain towns of western North Carolina the supply lines usually run through a vented crawlspace on block piers with nothing between them and the outside air but a sheet of foundation vent screen. Boone, Sparta and Spruce Pine sit high enough that a January night in the single digits is ordinary, not remarkable, and that changes what fails. Copper splits along the seam. PEX often survives a freeze and then leaks at a fitting where the ice pushed the ring. Once the wet section is found the plumber cuts back past the damage in both directions, since ice doesn't stop at the visible crack, and the pipe just outside the split has usually been stretched too. Then the joint goes back together in the same material family as what is already there, the water comes back on slowly to avoid a pressure spike, and every fixture gets run to purge air out of the lines. Before leaving, a good plumber walks the rest of the crawl for the next line that will do this, since one bad run is rarely alone.

Because a vented crawlspace is outdoor air with a floor over it. Heat from the rooms above barely reaches a supply line stapled to a joist near a foundation vent, and once wind moves through that space the pipe tracks the outside temperature within a few degrees.
It moderates the space, and plenty of older homes across the Blue Ridge were built expecting the owner to do it every fall. It is not a fix on its own, though. A line running tight against a rim joist on the north side can still freeze in a still, clear night when the temperature drops fast after sundown.
That depends entirely on your policy and how the loss happened, and the answer varies a lot between carriers. Contact your insurance provider before repairs get far along, and take photographs of the wet area and the failed section of pipe while everything is still as it was.
The repaired line gets flushed until it runs clear, since cutting into a pipe lets debris and air in. If your water comes from a spring or a private well and the break was below ground, testing afterward is worth doing, because a break under pressure can pull in soil water when the system drops.
